About this role
JavaScript, Graphic Design, PDF, Electronic Forms, Adobe InDesign, Adobe Acrobat · I need an experienced Adobe Acrobat and interactive PDF forms specialist to convert an existing six-page rehabilitation-clinic patient assessment and follow-up document into a professional fillable PDF.
The original document is A3 landscape and contains two A4-style sections on each page. Its existing layout, wording, tables and clinical structure must be preserved. This is primarily a technical fillable-PDF conversion, not a complete redesign.
Required features include:
• Single-line text fields • Multiline text fields • Date fields with consistent formatting • Dropdown lists • Checkboxes and radio buttons • Pain-scale selection from 0–10 • A practical method for marking pain locations on body diagrams • Repeated treatment-session fields • Therapist and physician signature fields • Logical tab order • Correct saving, reopening and printing of entered information • Field borders that do not appear unnecessarily when printing
The completed document should preferably use standard PDF AcroForm fields rather than XFA. Clinic staff must be able to fill, save and print it using the free Adobe Acrobat Reader without needing Adobe Acrobat Pro.
